Student Appointment Request Form

Students may fill out the following form to request an appointment with a school counselor. Once the form is filled out, a counselor will call you for your appointment. This form is for students to request a non-urgent appointment with a school counselor. If you need to report a safety concern for yourself or a friend, DO NOT fill out this form but immediately speak with a trusted adult and let them know you need to speak with someone immediately.

  • A lack of motivation
  • Irritability
  • Nervousness
  • Having a difficult time with engaging in virtual learning
