Hello Pulaski Families, 
Safety and health of our students is a top priority. With the upcoming break our goals is to ensure that we are all returning to school at the appropriate time. Below you will find a link to a form that will need to be filled out for any in person students if any of the following criteria are met:

• A student has tested positive for COVID-19 during the Thanksgiving Break
• A student has been identified as a close contact to someone who tested positive for COVID-19 
• A student is sick on either November 28th or 29th with COVID like symptoms 

If any of the above criteria are met fill out the form and a member of the Pulaski Contact Tracing team will contact you. The student is to remain in virtual school until they have been contacted and approved to return to in person instruction by a Pulaski staff member. Make sure you click the submit button at the end of the form. ​Additionally, please make sure you are notifying your student's homeroom teacher that you are filling out the form. ​

Hola familias de Pulaski,
La seguridad y salud de nuestros estudiantes es una prioridad absoluta. Con el próximo receso, queremos asegurarnos de que todos volvamos en el momento adecuado. A continuación, encontrará un enlace a un formulario que deberá completar cualquier estudiante en persona si se cumple alguno de los siguientes criterios:

• Un estudiante dio positivo por COVID-19 durante las vacaciones de Thanksgiving
• Un estudiante ha sido identificado como un contacto cercano con alguien que dio positivo por COVID-19
• Un estudiante está enfermo el 28 o 29 de noviembre con síntomas similares a COVID

 
Si se cumple alguno de los criterios anteriores, complete el formulario y un miembro del equipo de seguimiento de contactos de Pulaski se comunicará con usted. El estudiante debe permanecer en la escuela virtual hasta que un miembro del personal de Hesse se comunique con él y lo apruebe para regresar a la instrucción en persona.

Además, asegúrese de notificar al maestro de aula de su estudiante que está completando el formulario.​
