Spring Break Holiday Contact Tracing

​Good day Mercer Family,

Safety and health of our students and staff is a top priority.  With the upcoming Spring Break, we want to ensure that we are all returning at the appropriate time.  Below you will find a link to a form that will need to be filled out for any in-person student or staff member if any of the following criteria are met:

  • A student or staff member has tested positive for COVID-19 during the Spring Break
  • A student or staff member has been identified as a close-contact to someone who tested positive for COVID-19 
  • A student or staff member is sick on either April 3, 2021 or April 4, 2021 with COVID-19 like symptoms 

If any of the above criteria are met, please fill out the form and a member of the Mercer Middle School Contact Tracing team will contact you. The student or staff member is to remain in virtual school until they have been contacted and approved to return to in-person instruction by a Mercer Middle School staff member. 


Additionally, please make sure you are notifying your student's homeroom teacher or administrator that you are filling out this form.
