Immunization Requirements

Published on: 5/12/2021

Mercer Raider Family and Friends:

Children attending seventh grade and students age 11 or older entering Georgia schools are required to have one dose of tetanus, diphtheria, and pertussis (Tdap) vaccine and one dose of meningococcal (meningitis) vaccine.  These immunizations are necessary to complete Georgia Form 3231 for grades seven through twelve.

An audit of all 7th grade students’ immunization records will be performed by The Chatham County Health Department. Any student who does not have a 3231 (certificate of immunization) marked complete for 7th grade or higher, will be withdrawn from school.  Please make sure your child’s immunizations meet school requirements prior to August 2021.

Vaccines are safe and effective in protecting individuals from devastating and harmful diseases.

Information for parents on vaccine preventable diseases can be found at https://www.cdc.gov/vaccines/parents and http://www.gachd.org/.

Please contact your child’s physician if you are not sure if they have the required immunizations for school attendance.  You may also contact the Chatham County Health Department for answers to your questions regarding immunizations.
