Physical packet 2020-2021 updated.pdf

All athletes for the 2020-2021 season should complete a physical packet in its entirety.  Upon completion it should be handed to Coach King to put on file.  Attached is the physical packet for the 20-21 season. Physicals are good for 1 year from the date signed.  Students are not allowed to tryout or participate until packets are completed and on file.​
