Butler Elem Break COVID-19 Contact Form

​Hello Butler Families, 

Safety and health of our students is a top priority. With the upcoming break our goal is to ensure that we are all returning to school after the break at the appropriate time. Below you will find a link to a form that will need to be filled out for any in person students if any of the following criteria are met:

  • A student has tested positive for COVID-19 during the Winter Break
  • A student has been identified as a close contact to someone who tested positive for COVID-19 
  • A student is sick on either January 9th or 10th with COVID like symptoms 

If any of the above criteria are met, please fill out the form and a member of the Butler Contact Tracing team will contact you. The student is to remain in virtual school until they have been contacted and approved to return to in person instruction by a Butler staff member. 

Additionally, please make sure you are notifying your student's homeroom teacher that you are filling out the form.​
