 Incident Report Request

This is a formal request to the SCCBOEPD for a copy of a police accident or incident report 

Requests must be clear enough to enable the records unit to conduct a meaningful search. Please include as much descriptive information in the request as is available, such as case numbers, date ranges, addresses, names, etc. BOEPD may ask clarifying questions about the request in order to respond to the request fully and in a timely manner.

If you are requesting a report on behalf of another person, depending on your relationship to that person (the report subject), you may be required to provide a letter or release form signed by the report subject, authorizing the release of their data to you .  The requestor will be contacted if an authorization form is needed.
