Georgia Student Health Survey Is Now Open

​The 2023-2024 Georgia Student Health Survey, Georgia Parent Survey, and the Georgia School Personnel Survey is now open. The Georgia Student Health Survey (GSHS) is an anonymous, statewide survey instrument created by the Georgia Department of Education (GaDOE) and administered online each year to students in grades 3-12. Middle and high school students (grades 6-12) participate in the standard survey and elementary students (grades 3-5) participate in an abbreviated survey. Surveys will be completed by students at individual school sites. These surveys will contribute to the 2024-2025 School Climate Score and the district's DAS-REMI student wellness report. Specifically, survey results assist in the identification of safety, climate, and health issues that impact student achievement and help guide school prevention and intervention strategies.

 

Per the Protection of Pupil Rights Amendment (PPRA), parents/guardians may review the student survey items and have the option to “opt out" of survey participation by returning a signed consent form (see link below) to the student's respective school. See links to the survey items and form below:
